Solution

If you facing issue in submitting form on this site, after uploading photo and signature, then rename your photo and filename without putting space in between like abcphoto.jpg/png/jpeg or abcsign.jpg


Reason

if your photo name is "ash amend.jpg" like this, it is not reading spaces between file name of your photo and signature.

and it also not showing any warning.

and you will not able submit .
